Hello all,

Recently, there was an outbreak of diarrhea in the Indian city of Indore, which left 20 people dead and many many more hospitalised. Such incidents have also been reported in other parts of the country. I want to start a thread for discussing what all could have led to things going so out of hand. It's yet another tragedy that only when such terrible incidents happen do we start talking about and addressing the problem. 

I have penned down my thoughts in a LinkedIn post . As someone working at the intersection of governance, institutions, and sanitation, the most glaring reason for me seems to be weak institutions. But I am cognisant of the fact that it is but one part of the whole puzzle. I would like to know what you all think. How do we ensure systems that anticipate such risks and mitigate them? What all must be done to ensure that things actually change on the ground?
